Social Work Services
In addition to groups and support services on-site at the center, limited "home visits" by the social worker are available to the homebound elderly in effort to meet their goal to "age in place". The social worker also provides case management services, emotional support and counseling for issues including loss, caregiver's stress, reduced independence, relationship issues, depression, substance abuse, isolation and more. There is also assistance available to help guide the process of Advanced Directives, Powers of Attorney and organizing of personal affairs. Referrals made to local agencies and resources as needed.
If you need something to be notarized, please call first to schedule an appointment. Notary services are complimentary.
